Two clinical trials for “breathing lung” transplant enrolling

Gabriel Loor, MD, is the principal investigator for two “breathing lung” clinical trials—named INSPIRE and EXPAND—which are enrolling at Transplant Services at University of Minnesota Medical Center.

Enrollment continues for the INSPIRE phase III clinical trial. INSPIRE is a prospective, international, multicenter, randomized controlled trial comparing preservation of donor lungs using the Organ Care System Lung perfusion device versus standard cold flush and ice storage. Interim results demonstrated that donor lungs preserved using the Organ Care System Lung had significantly lower incidence of severe primary graft dysfunction (grade 3) after lung transplantation than did lungs that were preserved using cold storage. The results were announced at the International Society for Heart and Lung Transplantation 33rd Annual Meeting in Montreal.

Enrollment for the EXPAND clinical trial is now open. EXPAND is a phase III trial designed to evaluate the safety and effectiveness of the Organ Care System Lung in recruiting, preserving, and assessing donor lungs that may not meet current, standard donor-lung acceptance criteria for transplantation. The primary endpoint is the composite incidence of patient survival at 30 days post-transplant and freedom from primary graft dysfunction (grade 3) during the first 72 hours after lung transplantation.

New study published: Specialized intravenous insulin infusion protocol after pediatric TP-IAT

Overstimulation of transplanted islets by hyperglycemia can result in ß-cell loss after TP-IAT. To combat this tendency, Melena Bellin, MD, Srinath Chinnakotla, MD, and Sarah Jane Schwarzenberg, MD, and colleagues at University of Minnesota Masonic Children’s Hospital developed a specialized intravenous insulin infusion protocol for pediatric TP-IAT recipients that was shown to maintain euglycemia or near-euglycemia posttransplant. Their work is published in the November 2014 issue of Diabetes Technology & Therapeutics. 1
